Complaint on Unprofessional Conduct

A formal complaint may be submitted for an alleged offence under the Geologists Act 2008, the registration regulations or an applicable professional code.

Submission requirements

The form must be completed in full. The complaint should refer to the relevant provision of the Geologists Act 2008, the Geologists (Registration of Geologists) Regulations 2015 or the applicable professional code.

Identify the provision

State the section of the Act, regulations or professional code that is relevant to the complaint.

Complete the form

Provide the complainant's details and information about the geologist concerned.

Describe the conduct

Set out the alleged unprofessional conduct clearly. Use additional pages when required.

Attach evidence

Include relevant contracts, correspondence, reports and other supporting material.

Sign the declaration

Confirm that the information is correct to the best of the complainant's knowledge.

Submit to the Registrar

Submit the completed form and supporting documents in person. An acknowledgement of receipt will be issued.

Privacy

Personal information is protected under the Personal Data Protection Act 2010. Complaint details may be disclosed to the geologist concerned at the strict discretion of the investigating officer appointed by the Board. Signing the form records the complainant's agreement to this process.
