REINSTATEMENT OF REGISTRATION
1. Reinstatement under Section 35(2) of the Act
A registered geologist or practitioner whose name has been removed from the Register for failure to renew registration for a period of not more than three years shall be reinstated upon notifying the Registrar of their desire to be reinstated and upon payment of the prescribed fee. The application must include:
- Application form (Form G).
- Proof of payment for the reinstatement fee.
- CPD (Continuing Professional Development) record.
- Any other information required by the Board.
Only complete applications with all supporting documentation will be processed.
2. Reinstatement under Section 35(3) of the Act
A registered geologist or practitioner who fails to renew their registration for more than three consecutive years may be reinstated if the Board is satisfied with the reasons for reinstatement and upon payment of the prescribed fee. The application must include:
- Application form (Form H).
- Proof of payment for the processing fee (RM50.00).
- A letter explaining the reasons for failure to renew the registration.
- Any other information required by the Board.
Only complete applications with all supporting documentation will be processed.
3. Reinstatement under Section 35(4) of the Act
A registered geologist or practitioner whose name has been removed from the Register under an order of the Board pursuant to Paragraph 33(2)(d) and who has not appealed, or whose appeal has been dismissed or withdrawn, may apply for reinstatement after a minimum period of six months from the date of the revocation order, or the appeal decision/withdrawal date, whichever is later. The application must include:
- Application form (Form I).
- Proof of payment for the processing fee (RM50.00).
- A letter explaining the reasons for their desire to be reinstated.
- Any other information required by the Board.
Only complete applications with all supporting documentation will be processed.
